To help clients build their life skills, case managers create care plans that are based on the client’s ability to develop new skills. Strength-focused plans provide steps to train clients in the development of these skills. In this assignment, you will practice developing a care plan that uses a hypothetical client’s strengths.

Sample Service Plan

Use the S.M.A.R.T. goals that you wrote in this week’s discussion post as the basis for a hypothetical case plan for Hanna. Use the Sample Service Plan to create a short service plan using Hanna’s presenting problem and the goals to remove her barriers to treatment. Remember to base the goals on her strengths. Then, do some research to find at least three agencies in your local area that could provide these services.

Create a 5–6 slide narrated PowerPoint Presentation incorporating speaker notes incorporating the following:

What are Hanna’s barriers to treatment and how those could be prevented or minimized?
Predict Hanna’s success by explaining her strengths and relative weaknesses.
Write at least two S.M.A.R.T. goals to help her strengthen her self-sufficiency skills.
Compare different models and cite examples and factual/research support for the client focused model.
Give some examples of how her strengths can be used to help her overcome her relative weaknesses.
