Demonstrate the use of therapeutic communication, caring behaviors, and client self-determination when completing a health history and a wellness assessment.

Use the nursing process to develop a health promotion plan of care for a community client in one of the following age groups: infant ; toddler; preschooler; school age ; older adult. Selecting an adolescent age group
is also permitted with prior faculty approval.
Assignment Objectives
This assignment provides the opportunity to meet the following course learning outcomes:
1. Demonstrate the use of therapeutic communication, caring behaviors, and client self-determination when completing a health history and a wellness assessment.
2. Apply developmentally appropriate communication techniques and relational skills when interviewing a client.
3. Identify barriers to successful communication
4. Report patient safety issues and potential health risks in the performance of health assessment across the lifespan.
5. Apply adherence and motivational theories principles to the health promotion plan.
6. Assess factors contributing to health promotion and lifestyle choices
7. Create a suggested/recommended health promotion plan of care for a selected client.
8. Conduct an educational session on a selected health promotion topic for a pediatric or geriatric client.
