Mr. Lam is an 87-year-old diabetic with early stage Alzheimer disease. Currently he is living alone in his home. When his wife was alive she ensured that he ate properly, had monthly appointments at the chiropodist and that he maintained his daily glucose monitoring. Since his wife died 1 year ago Mr. Lam has not gone to the chiropodist, does not eat regularly and does not test his glucose level daily. He tells his family how lonely he is and how he just wishes his wife was here. His two grown children live in another state and although they notice that their father is more forgetful and fear for his safety; they cannot move closer to him or have him move in with them. This week Mr. Lam’s neighbor noticed he did not look well and told him to test his blood sugar. Mr. Lam was severely hypoglycemic and had to be rushed to the hospital. After being discharged to his children’s care it was suggested that the Lam family look at alternate living arrangements for their father.
