Tracer Methodology

Tracer methodology is used to identify performance concerns within an organization (Siewert, 2018). It is one technique that can be used by all healthcare organization departments to get a real-time snapshot of quality performance. Tracer methodology is used by The Joint Commission’s (TJC) on-site surveyors to follow patient care, services, or treatment processes (The Joint Commission [TJC], 2018). Individual tracers follow a selected patient through their care process (Siewert, 2018). These tracers are often a good selection for patients with specific diagnoses. System tracers follow a process in an organization (TJC, 2018). These tracers are often completed in an interprofessional meeting. There are three topics often covered in these meetings. The topics include data management, infection control, and medication management (TJC, 2018). A meeting about medication management within a hospital may include members from pharmacy, leadership, education, transport, nursing, and physicians (Siewert, 2018). This may occur in the planning as well as the evaluation stages of a QI project. The Program-specific tracers focus on a specific time interval with continuity of care (TJC, 2018). These can be focused on patient waiting times in the emergency department or patient transfers between departments. Handoff concerns are important with these tracers (Siewert, 2018). Tracers can provide data collection while planning a QI initiative, and they can then be repeated after implementation to collect the data to evaluate the effectiveness of the initiative.

Anna Garcia is a 67-year-old woman who is visiting her daughter from Puerto Rico. She suddenly experiences an episode of severe chest pain. Her daughter Rita, who recently delivered a healthy 8-pound newborn, calls for an ambulance, but is unable to accompany her mother to the hospital. Although Rita speaks English fluently, Anna Garcia speaks only Spanish and can understand only a little English. Anna is rushed to the nearest hospital where the staff struggles to communicate with her as best they can. In the emergency room Anna holds her hand over her chest and grimaces as the pain seems to grow more severe.

One of the ER nurses grabs Mr. Rodriquez, a member of environmental services, and asks him to assist by translating for the nurses and doctors caring for Mrs. Garcia, because it is 8 a.m. and the only Spanish-speaking medical interpreter, Mr. Sanchez, is not due in this morning until 10 a.m. Based on the EKG findings and first lab results, the staff believes that Mrs. Garcia is experiencing an They ask that Mr. Rodriquez explain to her that they are awaiting the results of other laboratory studies to confirm the medical diagnosis, but believe that she probably had a heart attack and will need further testing to confirm the medical staff’s suspicion. Mrs. Garcia becomes extremely anxious and begins to cry. The staff is baffled about what to do next.

Ms. Vera Talsford, a 65-year-old widow, mother of two daughters, and grandmother of two young boys, arrived in the ER of a busy metropolitan trauma center complaining of a severe headache with increasing pain, dizziness, and nausea with movement. After the physical examination, the physician ordered a CAT scan, which revealed a left occipital mass. Ms. Talsford was admitted with a brain tumor. During the next few days, MRI studies, a PET scan, and other tests revealed that Ms. Talford had lung cancer with metastases to her brain. A friend of the family was with the patient when the doctor spoke with her. He asked Ms. T, as everyone called her, if it was okay with the patient to speak about her situation while the visitor was present. Ms. T said that was okay with her. The doctor spoke encouragingly and told Ms. T that everything would be done to remove the tumor from her brain. Over several days, Ms. T made a slow neurological recovery from the surgery, but became more despondent, after being told about the severity of her illness. This is a continuation and finishing portion of the paper that has been created over the past 2 weeks (Order ID 3982809562). Paper is attached along with example, rubric, and template for appendix. Will be doing a virtual practicum so expected outcomes for project will not be with real students but with my idea of what the student’s response would be in the pre/post survey.

This plan follows a logical trajectory, starting with objectives. Use a separate paragraph or two for each of the following sections.

Using the Education Practicum Project template, the following headings are completed: Intervention and Evaluation Plan and subheadings: Objectives, Description of Practicum Site and Stakeholders, Change Model, Implementation Plan, and Evaluation. Objectives: State your long-term Objective. This is an objective that you have for the practicum site and mentor. When you leave your practicum, what should they be measuring or how should they follow up? Osteogenesis imperfecta (OI) is a genetic disorder of connective tissues caused by an abnormality in the synthesis or processing of type I collagen. It is also called brittle bone disease.
