Fishbone Diagram Project

Unit VI Assignment
Fishbone Diagram Project
Read the U.S. Chemical Safety Board investigation report of the 2007 propane explosion at the Little General Store in
Ghent, WV. The final report can be read/downloaded at the following link:
https://www.csb.gov/assets/1/20/csbfinalreportlittlegeneral.pdf?13741.
Additional information on the incident, including a video summary, can be found at the following link:
https://www.csb.gov/little-general-store-propane-explosion/
This is the same investigation report used to complete the assignments in Units IV and V.
Complete the assignment as detailed below.
Part I: From the information in the report and from the information you developed for the assignments in Unit IV and Unit V, create a fish bone diagram that illustrates the relationship between the causal factors and the accident. In your diagram, the backbone of the fish should represent the accident, and the big bones should represent the people, procedures, environment, equipment, and policies.
On a separate page, discuss what new information about the accident is revealed in the chart, and describe how that information.

Explain the three steps in the fault tree process. What does each level represent?

Your response should be at least 75 words in length.

Do you want to add a line from step 8 to step 10 to complete the call? If you turn the page to landscape orientation, it might be easier to follow the “natural flow of tasks from left to right” .

It’s implied that the performance context is when customers “call the call center,” like in the example on page 27, and the tools available are possibly this training, job aids, or utilizing the database, but naming them might make your instructional goal more specific. I tried to use the example from the text as a MadLibs-type exercise and just swapped out the words to match my learning module.

The project examples were a bit mixed, however several of them seemed to name each component.
From my perspective, the style and content of your diagram are clear and easy to understand, fulfilling the requirement that “the statement of each step must include a verb that describes an observable behavior” .

Do you want to add a line from step 8 to step 10 to complete the call? If you turn the page to landscape orientation, it might be easier to follow the “natural flow of tasks from left to right” . Stacking them vertically could be confused as indicating hierarchy; page 66 says, “all subordinate skills appear in boxes that are attached via lines coming from the tops and bottoms of boxes” .

1. This scenario is not uncommon, as many consumers have questioned the weight/ratio of the items they have received. With this said, it’s not a bad practice to run a test like this every so often to ensure the machines are correctly calibrated. Here is some additional information on why these tests are important: Hypothesis testing

Do you conduct tests like this at your employer? When may a test like this be applicable?

2. Using these statistics, we can indeed conclude that the machinery should be recalibrated. Creating a null and alternate hypothesis is one of the first steps when faced with a situation like this.Video on Hypthesis testing
