When undertaking the reflective task on the risk of serious harm activity please ensure that you use a model of reflection. This link provides an overview of a number of reflective models. This is not an exhaustive list and you are not restricted to what is contained here, use any model that works for you.
As this task is only 500 words you may wish to focus on one element of learning from the experience, for example, the impact of diversity/marginalisation on your risk assessment, reflection on your personal bias, using your professional discretion when not in agreement with an actuarial score.
